The cause behind this accident is stipulated to be chunks of insulating foam that broke away from its fuel tank during its launch two months ago. The reason behind this continuous loss may be the workers, who accidentally crushed or cut the section of foam during the launch. The Columbia faced a similar problem in 2003, despite two and a halh years of assurance that this was the safest tank ever built. As reported by AP, the shuttle will not fly again until the foam insulation problem is resolved - no sooner than spring, as repair work has been decelerated because of hurricanes Katrina and Rita.
